Add "clean all" button for removing stuff from form and local storage

It would be cool to have one button which would clean up the form and any saved form state from old values or incorrect input, that would be faster than clicking 'X' on every input field.

It doesn't need to be easily accessible, but it should be easily distinguishable from "Submit" button.
