docs(changelog): link SDK PRs with short labels and mark SDK items

This PR aligns CHANGELOG.md with SDK PR references and indicators.

Changes:

• Add [SDK] indicator to SDK-level items
• Replace sdk/# references with full SDK PR URLs using short labels like [SDK#222]
• Remove outdated SDK Package Versions block (git version not in sync with released version)

Notes:

• Lint: no bare-URL warnings; remaining multi-H1/duplicate-heading warnings are expected for a multi-release changelog
