This pull request adds a derived mapping table for the waveform numeric data. The CSV file contains
labeltoomop_concept_idandomop_concept_codeforLOINCcodes. The concepts were mapped to LOINC using the online tool SearchLOINC and Athena for OMOP codes. Mapping codes were reviewed and verified by @xborrat.
