Knex migrations not complete

The original_filename column in the library_items table isn't part of any of the knex migrations. I'm not sure if there are any others missing.

I was just having issues with my local environment and using bookmarks. I believe the issue is that there needs to be a primary key on library_item_id,time for the upsert's ON CONFLICT to work.

I'm not sure how to compare the schema of what's running on RDS with what's built with Knex, but it would probably be a good idea to make sure we get all these little inconsistencies figured out.
