Session rename (via /rename or /session + ctrl-r) not working

Description

Unable to rename sessions using either:

1. The /rename command
2. The /session command followed by ctrl-r

OpenCode version

1.0.11

Steps to reproduce

1. Open a session in OpenCode
2. Try method 1: Type /rename and attempt to rename the session
3. Try method 2: Type /session, then press ctrl-r to rename
4. Observe that neither method successfully renames the session
