This issue could be related to DICOM pixel data encoding. Please check:

1. The Transfer Syntax UID of your DICOM file
2. Whether the pixel data is compressed (JPEG, JPEG2000, etc.)
3. Try using cornerstoneWADOImageLoader for web-compatible DICOM loading

Note: cornerstone v2 is deprecated, consider migrating to cornerstone3D (https://github.com/cornerstonejs/cornerstone3D)
