Parallel tool calls rendering failures and ux rendering issue

What happened?

@NTaylorMullen reported two different issues:

• When approving parallel tool calls, the previously confirmed tool call disappears from the UX, and after the second confirmation, all tool calls vanish. Also, the rendering is a bit off with too many dots.
• The policy engine doesn't seem to respect the user's approval file, prompting for confirmation on simple, whitelisted commands like ls -F.
• The "Allow for this session" functionality was also reported as non-functional.

What did you expect to happen?

• The tool calls shouldn't disappear and the no weird ux
• policy engine should allow whitelisted commands
• allow for this session should work.
