t149: Auto-create GitHub issues when supervisor adds/dispatches tasks

Currently the supervisor creates tasks in SQLite but never creates corresponding GitHub issues. The TODO.md convention requires ref:GH#N on every task. Add gh issue create to cmd_add and cmd_batch, update TODO.md with the ref.
