[codex] Fix mobile title bar responsiveness

Summary

• Make the title bar wrap cleanly on narrow screens.
• Let the URL input shrink instead of forcing a fixed 560px width.
• Keep mobile action buttons compact and collapse desktop gutters so the conversation count is not clipped.
