chore: restructure CHANGELOG to follow Keep a Changelog format

Reformatted CHANGELOG.md to align with Keep a Changelog 1.0.0.

• Organized existing entries under standard sections (Added, Changed, Fixed, etc.)
• Preserved all version numbers and release links
• No content removed or modified
• Documentation-only change
